    • I will provide you with a superbill, which is a receipt that includes the necessary codes and details for you to submit to your insurance company for possible out-of-network reimbursement.

    • Reimbursement (if any) depends on your specific insurance plan’s out-of-network benefits.

  • If you’d like to check your out-of-network benefits, you can ask your insurance company:

    • Do I have out-of-network coverage for outpatient psychiatric services?

    • Is there a deductible I need to meet first?

    • What percentage of the fee is reimbursed?

    • Do you require any specific forms or prior authorization?

  • You may see my name appear in insurance directories as an in-network provider. That reflects my work with a separate employer and is not related to my private practice. My private practice operates independently with a different tax ID number (EIN) and is not affiliated with any insurance network.

  • The cost of an appointment depends on the type of appointment scheduled.

    Adult Evaluation: $550
    Adult Follow-up: $275
    Child/Adolescent Evaluation: $700
    Child/Adolescent Follow-up: $350

  • In the event of a no- show or late cancel, the full cost of the appointment will be charged.
